Care Net Affiliation Renewal time is here! From Feburary 1 through March 31st, follow the easy steps on this page to renew your pregnancy center's affiliation with Care Net to continue access to your affiliate benefits, including a library of free educational resources, a network of national trainers and center specialists, exclusive discounts with industry partners, continuing education through Centers of Excellence University, the credibility that comes with the Care Net name and our Commitment of Care and Competence, and so much more.

Gather your center statistics to submit. You can find the complete list of questions here, as well as FAQs on statistics and renewal here.

Submit your statistics and center information in the easy form below. Please note: once you start this form, you must complete the form in its entirety to avoid losing your work. We recommend that you review the sample list above to gather your information first, including your Affiliate Center ID number(s), and allow roughly 45 minutes to complete your renewal survey in full.

Sign the Certificate of Compliance. You will simply check the areas in which you are in or out of compliance with Care Net's standards.

Submit your Affiliation Renewal Fee – also in the form below! (No more two-step process in separate places, it's all here for you in one easy spot.)
After submitting your information, you will receive a confirmation email with a receipt for your payment. Then, continue to enjoy your affiliation with Care Net through exploring educational content on the (soon-to-be updated!) Affiliate Portal, contacting a Center Services Specialist, purchasing resources and training materials for your center thorugh CareSource, and more.
